The Collingswood Public School District has specific registration requirements for students who attend our schools. In addition to meeting residency requirements, the State of New Jersey also requires a physical examination by a doctor and certain immunizations. Each of these requirements is outlined on the page for the appropriate grade span.
To complete the registration process, please click the link that corresponds to the grade level your child will be entering. Each grade span includes slightly different forms. Please complete all forms prior to scheduling an appointment for registration. Appointments for students in Grades 1-12 may be scheduled through our District Registrar by calling Nichole Costello at 856-962-5700 ext. 1006 or email firstname.lastname@example.org.
