FERPA, PRA, and Surveillance Notice: The Family Educational Rights and Privacy Act (FERPA) (20 U.S.C. § 1232g; 34 CFR Part 99) is a Federal law that protects the privacy of student education records. The law applies to all schools that receive funds under an applicable program of the U.S. Department of Education.
The Protection of Pupil Rights Amendment (PPRS), 20 U.S.C. § 1232h, requires the Collingswood School District to notify you and allow you to opt your child out of participating in certain school activities.

Military Opt-Out Form
Under the federal No Child Left Behind Act, school districts are required to provide the names, addresses, and telephone numbers of high school juniors and seniors (i.e., 17 years and older) to the United States Armed Forces Recruiting Agencies, and other authorized agencies, as requested. However, the Family Educational Rights and Privacy Act and the No Child Left Behind Act mandate that parents be offered an option to withhold the release of this information each year. If you wish to withhold the release of your child's name, address and telephone number, you must complete the form below and return it to the school. The request to withhold the student information is applicable only to the current school year.
Statewide Assessment Schedule and Parental Notification of Standardized Assessments
In accordance with statutory requirements (N.J.S.A. 18A: 7C-6.6), school districts are required to annually notify parents or guardians by October 1 of any statewide student assessment or commercially developed standardized assessment that will be administered over the course of the school year.
